Express Pass – Phoenix Bears
This form is for ordering an Express Pass AFTER you have already submitted your Request Your Phoenix Bear form.
If you did not select Express Pass on your original request form but would now like to upgrade to Express, you may do so here. Please have your original Phoenix Bears Number ready, as it is needed for this form.
How Express Pass Works
- Express Pass fee: $50
- Express Pass families purchase their own base bear from our Amazon Wish List
- Once we receive your base bear, you will move to the Express Pass List for a much faster turnaround time
- Express Pass fees help cover any additional expenses for your Phoenix Bear and the remaining funds after that help Phoenix Bears provide sponsored bears to bereaved families in financial need
- We have a 2 Express Pass Bears limit per HUG, so everyone’s bears keep moving forward on the waitlist

Important Instructions
After ordering your Express Pass:
1. Visit the Amazon Wish List and purchase your base bear – if you have not already purchased it (at checkout in Amazon, select the Phoenix Bears shipping address)
2. Include your original “Request Your Phoenix Bear” Number & your angel baby’s name in the Amazon gift receipt note
